PROGRESS STUDY: IMPROVING HIV CARE THROUGH THE IMPLEMENTATION OF PATIENT REPORTED OUTCOME MEASURES (PROS) WITHIN ROUTINE PATIENT MANAGEMENT

Background

PROs integrated within routine clinical care have demonstrated multiple benefits for patients and providers:

  • Increased identification of complex health and behavioural issues
  • Improved patient-provider communication, collaboration and patient involvement.
  • Improved patient outcomes

This project is a prospective pre-test/post-test study design with a 15-month timeline and a samples of 1600 patients across two sites. The study aims to improve patient care by supporting the adoption of actionable PROs as part of routine HIV management.

The study aims to generate a 3-part solution;

  • Provide compelling evidence of the benefits of PROs to routine care (Evidence Review and Summary document).
  • Provide guidance on ‘how to’ implement PROs in a clinic (toolkit).
  • Provide evidence of implementation within routine HIV care (PROgress site study outcomes).

Primary objectives

  • Assess patient, provider, and broader stakeholder perceptions of the value, barriers, experience, and acceptability, when integrating PROs as part of a routine appointment; Evaluate impact on clinic.
  • Assess any changes in clinical/medical practice (e.g. identification of depression, substance abuse, treatment compliance, referral patterns).

Conclusions

Successful integration of PROs in primary HIV care requires key stakeholder buy-in through clear demonstration of value added and involvement in domain selection, PRO feedback development, risk protocol development, and piloting. Designation of key personnel to train new staff and address issues at the patient, flow, and IT levels are also critical to successful implementation.
